Introduction

The Affordable Care Act aims at encouraging the insurers to effectively compete with a focus on the price and quality of service provision and to discourage them from competing to enroll the healthiest participants (Faguet, 2013). Today, South Carolina has many at risk population facing medical conditions like COPD, Cancer, and diabetes among others. This has changed the way healthcare organizations provide their services. Most of the healthcare organizations are now working to identify the people’s needs while putting into consideration various factors that influence their health. The advantage of this is that it allows the healthcare care teams to develop care plans that truly reflect the need of the patients in order to address their needs.

Most healthcare organizations have opted to segment the population using various health risks, which allows the leaders to commit resources towards the provision of the services their population subsets depending on the identified needs. Medical researchers have noted that some key strategies being used by providers in managing these population include the high levels of involvement of the patients and families in the decision making process, forming new communities and payer partnerships, and using advanced technology for collecting, analyzing and reporting the data f the at risk population. Healthcare providers are investing high level infrastructure and systems that are necessary in managing the health of the communities and populations in which they serve (Gitman, Joehnk, & Billingsley, 2013). Much is being done to help at risk populations who often require more attention and care during an emergency.
