1. Discuss strategies to improve patient safety in the institution and nursing professional practice

From the videos watched, it is evident that most people think of hospitals as cathedrals of hope. Nonetheless, these hospitals are faced with challenges of the failing support systems which are equated to the invisible enemies that cannot sleep, leading to medical errors. These errors as noted have cost the lives of about 100,000 Americans every year since 1999 and often double if the infections patients get from hospitals are included. Dr. Charles Denham and other professionals have taken a lead in making changes by improving the patient safety as individuals and for institutions too. For individual professional nurses, patient safety can be improved by compiling to the codes of ethics to remain competent in their work. Individual nurses must be careful when labeling medication for their patients to include all information to avoid costly errors. Dennis Quaid’s twins were wrongly diagnosed due to tragedies of labeling of medications that were computerized by the physician orders.

On an institutional level, it is portrayed from the videos that effective leadership is essential for patient safety as leadership; safe practices and technology form the most important elements of patent safety (Durham & Dwyer, 2014). Leaders must confront fears of adapting technology for safety care. A collaborative approach that enhances teamwork and communication among all nurses and physicians is also important in achieving patient safety in daily operations. Hospitals need to put in place more patient-safety initiatives and further take time to evaluate their success. This can be achieved through the inclusion of safety in hospitals via the measures of performance with balanced scorecards included. Hospitals need to develop innovative training in work-related training for patient protection. Medical centers must maintain up-to-date patient-safety requirements and methods.


2. How to participate in creating a culture of safety in the work environment

To create and maintain a culture that advocates for patient safety in the hospitals, there is a need shift the perception of blaming medical errors and mistakes to individual practitioners but rather, foster the general attitude that views mistakes as chances to learn more and make improvements in care provision (Wong & Beglaryan, 2004). Processes of improving quality must also be established in every hospital. It is also important to make patient safety a crucial part of the performance assessment and professional accountability.
